Student Registration

For families registering their children for school, please be advised that registration is not complete until both the completed registration form and payment have been submitted. If online payment cannot be made, please contact the District to make alternative payment arrangements.

Please add any new to the district students in your existing household parent portal.

Residency Requirements
Families new to CCSD 46 must provide a birth certificate and three (3) residency proofs which can be uploaded during the application process. These documents can also be emailed to [email protected].

    Category 1 – (one document showing proper address) ISSUED WITHIN THE LAST 60 DAYS

    • Current mortgage papers or statement
    • Current original lease agreement with signatures
    • Current real estate tax bill
    • Affidavit from homeowner attesting registrant is living with the owner at no cost. Affidavit must be signed by the homeowner, notarized, and accompany of photo ID of the homeowner attached. Affidavits are only good for one school year.

    Category 2 – (one document showing proper address) ISSUED WITHIN THE LAST 60 DAYS

    • Utility bill (gas, electric, water)
    • Internet/Cable TV bill

    Category 3 – (one document showing proper address) ISSUED WITHIN THE LAST 60 DAYS

    • IL Driver’s License/IL State ID Card
    • Voter registration card
    • Residence insurance document
    • Vehicle registration
    • Credit/debit card statement

   

For families of continuing students (and new to the district siblings), one (1) proof of residency is required which can be uploaded during the application process. This document can also be emailed to [email protected]

Registration Fee Schedule

Health Requirements
The following State-mandated health requirements must be met prior to the first day of school. All Physical Exam forms must be submitted to the school office on or before the first day of school for children to attend school.

New Students
Physical, Dental, Vision exams, Record of Immunizations
Kindergarten
Physical, Dental, Vision exams, Record of Immunizations
Second Grade
Dental exam
Sixth Grade
Physical, Dental exams, proof of Tdap and MCV4 (meningococcal vaccine)
Kindergarten – Eighth Grade
Proof of three doses of Hepatitis B

Additional Information

Community Consolidated School District 46 charges each student a registration fee. This fee is used to help offset a portion of the cost of materials and supplies that are used by students. These include workbooks, subscriptions, copy paper for materials distributed in class, science materials, paint, glue, markers, Chromebooks, software, and other related expenses.
